Request a second opinion or transfer of care

We ask that you:
Share honest and complete information about your health
Follow the care plan we develop together
Treat our staff and fellow patients with courtesy and respect
Arrive on time for appointments or cancel in advance if needed
Ask questions if you don’t understand your treatment
Pay co-pays and balances as agreed
Use medications and telehealth tools as directed
