Your Rights

As our patient, you have the right to:

  • Receive care that protects your privacy and confidentiality

  • Get clear explanations about your health and treatment

  • Participate in decisions about your care

  • Decline treatment if you choose

  • Access timely care, including telehealth visits

  • Request a second opinion or transfer of care

  • File a complaint without fear of retaliation

Your Responsibilities

We ask that you:

  • Share honest and complete information about your health

  • Follow the care plan we develop together

  • Treat our staff and fellow patients with courtesy and respect

  • Arrive on time for appointments or cancel in advance if needed

  • Ask questions if you don’t understand your treatment

  • Pay co-pays and balances as agreed

  • Use medications and telehealth tools as directed
